Notice of Intent To Request Approval From the Office of
Management and Budget of a New Information Collection Activity, Request
for Comments: Specific Release Form
AGENCY: Federal Aviation Administration (FAA), DOT.
ACTION: Notice and request for comments.

SUMMARY: The FAA invites public comments about our intention to request
the Office of Management and Budget (OMB) to approve a new information
collection. The information garnered from a signed Specific Release
form will be used by FAA Special Agents to obtain information related
to a specific investigation.
DATES: Please submit comments by April 14, 2009.

Affected Public: A total of 270 respondents.
Frequency: The information is collected on occasion.
Estimated Average Burden per Response: Approximately 5 minutes per
response.
Estimated Annual Burden Hours: An estimated 22.5 hours annually.
Abstract: The information garnered from a signed Specific Release
form will be used by FAA Special Agents to obtain information related
to a specific investigation. That information is then provided to the
FAA decision making authority to make FAA employment and/or pilot
certification/revocation determinations.

minimize the burden of the collection of information on respondents,
including the use of automated collection techniques or other forms of
information technology.
