Sec. 4011.9  Manner of issuance of notice.

    The Participant Notice shall be issued by using measures reasonably 
calculated to ensure actual receipt by the persons entitled to receive 
it. It may be issued together with another document, such as the summary 
annual report required under section 104(b)(3) of ERISA for the prior 
plan year, but must be in a separate document.
