If the registration of any purchaser terminates (because the purchaser dies, ceases legal existence, discontinues business or professional practice, or changes his name or address as shown on his registration) or is suspended or revoked pursuant to Sec. 1301.36 of this chapter as to all controlled substances listed in Schedules I and II for which he/she is registered, he/she shall return [[Page 72]] all unused order forms for such substance to the nearest office of the Administration.
